Schedule or Change an Appointment
- New Patient. If you are a new patient, please call the desired SKI Location to schedule your appointment. We recommend you also download and complete the New Patient Packet prior to your first appointment.
- Existing Patient. You may schedule, change or cancel an appointment through the SKI Patient Portal or by calling your SKI Location.
