New Patient
If you are a new patient, please call the desired SKI location to schedule your appointment. We recommend you also download and complete the New Patient Packet prior to your first appointment.
Existing Patient
You may schedule, change or cancel an appointment through the SKI Patient Portal or by calling your SKI location.
General Inquiries & Feedback
If you have a general question or feedback you would like to share, please fill out the form below. A member of the SKI team will reply to you within 72 business hours. Please do not include personal health information, personal identifiers such as social security number, birthdate, etc., or credit or debit card information. If you have specific medical questions about your treatment or need to make a payment, please call your SKI physician’s office or contact us through the secure SKI Patient Portal.
